Louisiana Health and Rehab Options, Inc. (LHRO) ”Where Growth Begins,” provides an extensive array of quality waiver services to developmentally, intellectually, and physically disabled individuals across the lifespan from children to the elderly.  The community and home-based waiver programs are designed to enhance greater levels of independence, functionality, and an overall higher quality of life for Medicaid recipients who would otherwise require institutional care. Waiver programs are strategically targeted to age and degree of impairment or disability. Only individuals who have a waiver for home-based services are eligible for LHRO services, however LHRO is experienced and expertly skilled with assisting and often expediting the waiver process.

In-Person Assessment

LHRO waiver services are first and foremost established on the foundation of relationship.  Your understanding of us and our program of services and our understanding of you and your loved ones is vital.  The ultimate purpose of the interview is to assess your family’s particular needs in order to tailor an Individualized Plan of Care ensuring the selection of the most compatible Direct Service Professional.

Direct Service Professional (DSP) Interview

After the meeting with you and your family, LHRO Individualized Program Management Team devises your loved one’s Individualized Plan of Care and identifies the best DSP. 

LHRO then schedules an appointment for an introduction/interview for you and your loved one and the proposed DSP to ensure compatibility, comfort, and confidence.

Growth Begins

Once the Individualized Plan of Care and Direct Service Professional are agreed upon, LHRO services for your loved one begin.  This is the beginning of the journey toward a new life of maximized independence with dignity regardless of  the degree of your loved one’s impairment or disability.

Direct Care Staff

LHRO Direct Services Professionals DSP’s must:

  • Have prior direct care experience

  • Undergo a criminal background check (0 tolerance)

  • Undergo 16 hours of training annually including:

    • service recipients’ rights and responsibilities

    • DSP’s responsibilities

    • infection control

    • recipient care

    • medication monitoring and administration

    • disaster preparedness, etc. 

Eligibility Criteria

  • Must be a Medicaid recipient or Medicaid eligible

  • Must meet nursing facility level of care

  • Must meet the Louisiana definition of developmental disability (when applicable)
