The following Eligibility Requirements
must be met in order for Waiver Services to be
provided:
The individual has been determined mentally
retarded or developmentally disabled as
defined by the Division of Mental
Retardation and Developmental Disabilities.
The individual has been institutionalized and is
at risk of becoming institutionalized or would
otherwise require ICF/MR level of care, unless
eligible to participate in Medicaid Home and
Community-Based Waiver Services.
